Holly

This is a composite story, meaning that it combines details from several real-life events, people, or situations into a single story or case study.

Holly, a single mother of two boys, Kai (13) and Logan (9), had been diagnosed with breast cancer and was undergoing treatment. While her condition had remained relatively stable for the previous nine months, her health had recently begun to deteriorate. As a result, there were growing concerns about the additional caring responsibilities the children were taking on, their overall wellbeing, a decline in school attendance, and the lack of clear plans for their future should Holly's health worsen.

A Family Group Conference (FGC) Coordinator began working closely with Holly, her wider family network, and the professionals involved in supporting her and her children. As part of this work, the Coordinator took time to understand Holly, Kai and Logan's wishes, feelings, hopes and concerns.

The initial conference enabled Holly, her wider family and invited professionals to develop a practical support plan to help the family manage Holly's increasing care needs. Responsibilities were shared across the wider family network, reducing inappropriate caring expectations on Kai and Logan and ensuring appropriate support services were in place. Both boys' schools were involved, enabling them to access additional support while continuing their education.

Sadly, following the Family Group Conference, Holly was informed that her treatment was no longer working and that she was approaching the end of her life. As a result, she made the decision to move into hospice care.

A second Family Group Conference was arranged promptly to help the family plan both for the coming months and for the longer-term future. The Coordinator worked closely with hospice staff to ensure the meeting could take place on site, enabling Holly to attend in line with her wishes. Recognising that Holly's health might prevent her from participating on the day, the Coordinator spent time with her beforehand to explore her views and wishes so they could be shared with the group if necessary.

The process created an opportunity for Holly to express her wishes regarding both her own care and the future of Kai and Logan. At the same time, the Coordinator ensured that both boys were able to participate in ways that felt comfortable and appropriate for them, keeping their voices at the centre of decision-making.

The Conference enabled the family to have open and honest conversations about what now that Holly was no longer able to care for her sons. Together, they developed clear plans, including shared responsibility for emotional and practical support during Holly's remaining care, long-term living arrangements for the children with their grandmother, arrangements to maintain school attendance and routines, and clear communication pathways with professionals.

The Family Group Conference provided clarity, stability and reassurance during an exceptionally challenging period. By bringing the family together to plan ahead at both key stages, the process strengthened family support networks, reduced uncertainty and ensured that Kai and Logan had a clear understanding of who would care for and support them in the future. Most importantly, it enabled decisions to reflect Holly's wishes while keeping the children's wellbeing, stability and long-term needs at the heart of planning.
